For referrers
Refer a patient
Case managers, care coordinators, support coordinators and GPs can refer by phone or email. We’ll contact the patient to book, at the Villawood clinic or at home.
How to refer
Call us, or email the referral to contact@sydneyclinicalphysio.com.au. Please include:
- the patient’s name and contact number, and who we should call
- clinic or home visit, and the suburb for a home visit
- how the physio is funded, with the claim, plan or approval details you have
- your name and contact details, so we can keep you updated
Please only send the patient’s details with their consent. We take the clinical history directly with the patient.
